Proposals should include all of the items listed below and organized based on this section outline.
1.     Application Checklist
2.     Narrative Statement and Worksheets
3.     Letters of Institutional Commitment:
a.    Medical School Dean and/or Associate Dean for Medical Student Education or equivalent * ; and
b.    Physician leader of the palliative care clinical program and
c.     Medical student clerkship/education director of the sponsoring/lead department. 
 
* Letter must indicate permission for the principle investigator to review the school’s M4 Graduation survey data dealing with palliative/end-of-life care, ethics and pain management.
