Learn about Vestibular Rehabilitation

If you wish to be evaluated in the clinic, please call (414) 805-5625. In order to best serve your individual needs, we ask that you complete a dizziness questionnaire (PDF). This form will be reviewed by our vestibular experts to determine the best plan of care. Once the form is received and reviewed we will call you to set up an appointment with what we feel would be the best initial step in your evaluation.

Although dizziness is commonly felt to be a condition of the ears, it is more often than not, a symptom of other medical conditions. Based upon your questionnaire we may recommend an initial evaluation by different professionals such as a neurologist, cardiologist, endocrinologist, pulmonologist, or behavioral psychologist. If you have current issues with high blood pressure, headache/migraine, diabetes, anxiety, depression or sleep apnea we recommend you have those conditions thoroughly evaluated and treated by your current medical team as these are common causes of dizziness.

At the time we call, you will be informed whether testing may or may not be required prior to the visit. Please forward any records specifically related to your history of dizziness such as hearing tests, balance tests, CT or MRI scans, operative reports, and office notes along with the history form.

We look forward to serving you!
