The Health Equity and Urban Clinical Care Partnerships Division was established in July 2010 and is one of the essential components of the Medical College of Wisconsin's (MCW) Institute for Health and Society. The Health Equity Division supports health promotion for underserved populations through:
• enhancing affiliations with other health systems • forging partnerships with federally qualified health centers (FQHCs) • developing models that uniquely establish a Partners in Health Model • fostering partnerships with community based groups/organizations, policy makers, and individuals within the private sector

Milwaukee continues to rank as one of the most segregated urban cities in the United States. This distinction, along with other social determinants of health, creates an environment that fosters inequitable health for vulnerable populations.
