Alumni Transcripts

A transcript is the formal documentation of enrollment and academic course work. This official document bears the signature of the Registrar, the school seal, the date of issue and is printed on security paper.

“Official” transcripts are sent directly to a requesting agency, e.g., residency program directors, educational institutions, hospital staff offices or licensing boards and are not issued directly to graduates or students.

Due to the confidential nature of this information, we do not fax or email official transcripts.

Please note: If you were enrolled in medical school prior to 1968 you will need to contact Marquette University to order an official transcript. You can reach the MU Registrar’s office by either calling (414) 288-4000 or online at Marquette Center: Grades and Transcripts site.
 

As dictated by Federal law, information regarding academic records can be released only upon the written consent of the student. Therefore, it is necessary for you to submit a written and signed request form to our office. Requests are usually processed within 2-4 business days upon receipt. However, during the months of April – July it may take longer due to extremely heavy year-end volume. Transcripts will not be released if there is a ‘hold’ on a graduate or previously enrolled student record (typically due to loan re-payment issues). Please contact the office of Sponsored Programs at jsunby@mcw.edu or (414) 955-8233 to obtain information regarding financial hold issues.

  

Nursing Transcripts

Medical College of Wisconsin School of Nursing opened for classes in August 1981, offering a two-year upper division baccalaureate completion degree program for registered nurses. In August 1983, additional classes were offered for nurses looking to earn a graduate degree in nursing. The last BSN and MSN graduating class from the Medical College of Wisconsin School of Nursing was in 1987.  Those students who attended or graduated from these programs can contact the MCW Office of the Registrar at (414) 955-8733 for transcript ordering instructions.

 

The students who attended the Milwaukee County School of Nursing program should contact the Milwaukee School of Engineering (MSOE) School of Nursing, Records Department for transcript request information. MSOE is located at 1025 N. Broadway, Milwaukee, WI 53202-3109 or contact them at (414) 277-7215 for further information.

Licensure and Doctor of Medicine Documentation

As dictated by Federal Law, information regarding academic records can be released only upon the written consent of the graduate.

Licensure:

The Office of the Registrar is happy to assist graduates with documentation for licensure applications. Specific inquiries regarding licensure should be referred to the state in which you are applying. Every state has different requirements for licensing as well as additional specific instructions when utilizing the Federation Credentials Verification Service in Euless, Texas. State licensing boards may require a specific form be completed and also require other medical school documentation such as an official transcript or medical diploma certification.

If a state requires a medical education verification form (receipt of MD and/or dates of attendance) be completed, please forward the form to the Office of the Registrar, 8701 Watertown Plank Road, Milwaukee, WI 53226. Be sure to include your signed release/consent with the form as well as your contact information. Please do not complete any part of the form unless instructed to do so by the Board. The Office of the Registrar will complete and mail the form. There is no charge to the student or graduate.

For those states which require an official transcript with or without a license application, please complete the Doctor of Medicine official transcript request form, paying special attention to the Board’s address, your personal information, signature, and mail with your payment to the Office of the Registrar.

If the state requires a notarized or certified copy of your diploma, you must provide us with a photocopy of your diploma. We do not keep diploma copies on file. Photocopies must be black and white, 8-1/2 x 11, with all wording and signatures clearly visible.

Due to the confidential nature of this information, we do not “Fax” these items.

Verifications:

It is common for graduate medical programs, medical offices and third-party providers to verify dates of attendance and/or degrees for graduates. The Medical College of Wisconsin has authorized the National Student Clearinghouse to act as its agent for all verifications of current student enrollment and degrees. Please refer third parties to the Clearinghouse or contact them by phone at (703) 742-4200.

Dean’s Letter/Medical Student Performance Evaluation:

The Office of the Registrar retains each medical school graduate’s Dean’s Letter/Medical Student Performance Evaluation (MSPE). A current or former student who wishes to authorize the release of his/her Dean’s Letter/MSPE and transcript must complete the appropriate Request for Official Academic Transcript form.
