As dictated by Federal Law, information regarding academic records can be released only upon the written consent of the graduate.
The Office of the Registrar is happy to assist graduates with documentation for licensure applications. Specific inquiries regarding licensure should be referred to the state in which you are applying. Every state has different requirements for licensing as well as additional specific instructions when utilizing the Federation Credentials Verification Service in Euless, Texas. State licensing boards may require a specific form be completed and also require other medical school documentation such as an official transcript or medical diploma certification.
If a state requires a medical education verification form (receipt of MD and/or dates of attendance) be completed, please forward the form to the Office of the Registrar, 8701 Watertown Plank Road, Milwaukee, WI 53226. Be sure to include your signed release/consent with the form as well as your contact information. Please do not complete any part of the form unless instructed to do so by the Board. The Office of the Registrar will complete and mail the form. There is no charge to the student or graduate.
For those states which require an official transcript with or without a license application, please complete the Doctor of Medicine official transcript request form, paying special attention to the Board’s address, your personal information, signature, and mail with your payment to the Office of the Registrar.
If the state requires a notarized or certified copy of your diploma, you must provide us with a photocopy of your diploma. We do not keep diploma copies on file. Photocopies must be black and white, 8-1/2 x 11, with all wording and signatures clearly visible.
Due to the confidential nature of this information, we do not “Fax” these items.
It is common for graduate medical programs, medical offices and third-party providers to verify dates of attendance and/or degrees for graduates. The Medical College of Wisconsin has authorized the National Student Clearinghouse to act as its agent for all verifications of current student enrollment and degrees. Please refer third parties to the Clearinghouse or contact them by phone at (703) 742-4200.
Dean’s Letter/Medical Student Performance Evaluation:
The Office of the Registrar retains each medical school graduate’s Dean’s Letter/Medical Student Performance Evaluation (MSPE). A current or former student who wishes to authorize the release of his/her Dean’s Letter/MSPE and transcript must complete the appropriate Request for Official Academic Transcript form.
