Office of the Registrar

The Office of the Registrar houses and manages activities related to the academic records of all currently enrolled medical students and medical school alumni. It is located within the Office of Academic Affairs suite at the south end of the third floor, in the Medical Education Building. Hours of operation are 8:00am to 5:00pm, Monday through Friday, or by appointment.

Academic Records

Contained in the academic file are copies of grades/evaluations submitted by each course director, correspondence pertinent to academic honors, leaves-of-absence, actions taken by the Academic Standing Committee and some but not all extra-curricular activities.


M-1 and M-2 daily schedules per academic year; medical school academic calendar for all years.

A replacement diploma may be ordered from the Office of the Registrar in the event that the original diploma has been lost or destroyed.

Enrollment Information

Requests for written verification of medical student enrollment at the Medical College of Wisconsin can be made by mail or in person. There is no cost to the student for this service.

The Academic Standards of Progress policy for students enrolled in the M.D. program can be found in the MCW Student Handbook.


The Office of the Registrar is happy to assist graduates with licensure applications. Specific inquiries regarding licensure should be referred to the state in which you are applying.

Registration Process

Student registration at the Medical College of Wisconsin takes place once annually, at the beginning of each academic year.

Transcripts-Currently Enrolled

A transcript is the formal documentation of enrollment and academic coursework. This official document bears a raised school seal, the date of issue and the signature of the Registrar.

Visiting Students

Visiting Students will be placed in available courses during the first week of June and will be sent a notification via email.
