Licensure and Doctor of Medicine Documentation
As dictated by Federal Law, information regarding academic records can be released only upon the written consent of the graduate.
The Office of the Registrar is happy to assist graduates with documentation for licensure applications. Specific inquiries regarding licensure should be referred to the state in which you are applying. Every state has different requirements for licensing as well as additional specific instructions when utilizing the Federation Credentials Verification Service in Euless, Texas. State licensing boards may require a specific form be completed and also require other medical school documentation such as an official transcript or medical school diploma certification.
If a state requires a medical education verification form (receipt of M.D. and/or dates of attendance) be completed, please forward the form to the Office of the Registrar, 8701 Watertown Plank Road, Milwaukee, WI 53226. Be sure to include your signed release/consent with the form as well as your contact information. Please do not complete any part of the form unless instructed to do so by the Board. The Office of the Registrar will complete and mail the form. There is no charge to the student or graduate.
For those states which require an official transcript with or without a license application, please complete the Doctor of Medicine official transcript request form, paying special attention to the Board’s address, your personal information, signature, and mail with your payment to the Office of the Registrar.
If the state requires a notarized or certified copy of your diploma, you must provide us with a photocopy of your diploma. We do not keep diploma copies on file. Photocopies must be black and white, 8-1/2 x 11, with all wording and signatures clearly visible.
Due to the confidential nature of this information, we do not “Fax” these items.
Graduate Medical Education offices, medical staff offices, and third-party providers typically require medical education verification (receipt of M.D. and/or dates of attendance) for employment purposes. The graduate or outside organizations may request medical school verification by sending or faxing a written request to the Office of the Registrar. Mail the request with your signed consent form to Office of the Registrar, 8701 Watertown Plank Road, Milwaukee, WI 53226 or fax both documents to Office of the Registrar (414) 955-0117. Verifications are completed in the order in which they are received.
Dean’s Letter/Medical Student Performance Evaluation:
The Dean’s Letter/Medical Student Performance Evaluation (MSPE) is kept on file for every graduate and will be sent when required for licensing, fellowship application, or employment purposes. To order, please indicate via a checkmark on the Transcript Request form. There is no charge to the graduate for sending the Dean’s Letter/MSPE whether or not an official transcript is included.
