A transcript is the formal documentation of enrollment and academic course work.  This official document bears the signature of the Registrar, the school seal, the date of issue and is printed on security paper. 

“Official” transcripts are sent directly to a requesting agency, e.g., residency program directors, educational institutions, hospital staff offices or licensing boards and are not issued directly to graduates or students.

Due to the confidential nature of this information, we do not fax or e-mail official transcripts.

Please note:  If you were enrolled in medical school prior to 1968 you will need to contact Marquette University to order an official transcript.   You can reach the MU Registrar’s office by either calling (414) 288-4000 or online at

As dictated by Federal law, information regarding academic records can be released only upon the written consent of the student.  Therefore, it is necessary for you to submit a written and signed request form to our office.  Faxed requests will not be accepted.

Requests are usually processed within 2-4 business days upon receipt.  However, during the months of April – July it may take longer due to extremely heavy year-end volume. 

Transcripts will not be released if there is a ‘hold’ on a graduate or previously enrolled student record (typically due to loan re-payment issues).  Please contact the office of Sponsored Programs at or (414) 955-8233 to obtain information regarding financial hold issues.

There is a $5.00 charge for production and mailing of official transcripts.  The charge for “unofficial” transcripts (no Registrar signature, no seal, no institutional letterhead) is $2.00.

If there is a need for a single rush transcript (official or “unofficial”) without additional documents included, i.e., a licensing form, MSPE or Dean’s Letter, a $10.00 fee will be charged.  If a rush transcript needs to be delivered to a U.S. address by overnight courier, an additional $25.00 delivery fee will apply.  Overnight international delivery requests will require the rush fee ($10.00) and an additional $50.00.

Payment may be made in person in the Registrar’s office or by mail (check only – made payable to the Medical College of Wisconsin).  We apologize that we are not currently set up to take credit cards or to make payments online.
