Mission Statement

The Special Needs Program (SNP) at the Medical College of Wisconsin and Children's Hospital of Wisconsin offers comprehensive care coordination for medically complex and fragile children with chronic conditions.

Our goal is to help children and families receive the best possible care in and out of the hospital.  We partner with patients, families, primary care providers, and other health care providers to improve care coordination. 

Patient Referrals

Children and youth, age 21 and younger, with multiple medical needs may be referred to the Special Needs Program. Families, caregivers, providers, schools, or community agencies may refer a child to the program.
