Notice of Privacy Practices

The Medical College of Wisconsin and its employees and other workforce members use and disclose protected health information only as permitted or required by the Privacy Rule, State and Federal law, and in a manner consistent with the Notice of Privacy Practices.  When using or disclosing protected health information or when requesting protected health information from another covered entity, MCW makes reasonable efforts to limit protected health information to the minimum necessary to accomplish the intended purpose of the use or disclosure.

Payment in full is requested by the due date on your statement.  For your convenience, we accept check, Visa, MasterCard, Discover, or American Express payments by phone and online.  Failure to remit payment in full may result in the placement of any unpaid balance with an outside collection agency.  If payment cannot be made in full, please contact customer service to discuss a payment plan.

The balance due on your statement may involve multiple invoices.  Payments made that satisfy an invoice balance will not appear on your next statement.  We recommend that you keep statement record information together for future reference.

Insurance Coverage:

Our billing office files health insurance claims on the patient’s behalf for most services.  We also submit claims to secondary insurance carriers once primary insurance benefits have been resolved.  We rely on you to provide accurate insurance information and to report any insurance changes directly to this office.

If your insurance company does not respond to our claim, we will expect payment from you.  Should you resolve the reason for nonpayment after contacting your insurance company, we ask that you notify customer service with the details.

If you have insurance you may still be responsible for some portion of the charges as there could be a copayment, coinsurance, deductible, other non-covered charge, etc.  Some health insurance plans have different coverage for a hospital based outpatient clinic.  The majority of our physicians practice at a hospital based outpatient clinic, which is not a doctor’s office.  Health insurance benefits vary; therefore if there is a question about how your claim was processed, please contact your insurance company and/or your employer.

Please remember the ultimate financial responsibility for payment lies with the responsible party, not with the insurance company.

Patients with Traditional Medicare Coverage:

We accept Medicare assignment and will submit your claims to Medicare and any supplemental insurance. You will be responsible for any deductible, coinsurance and amounts not covered.
