Mi salud - Mi vida:  Health Promoter Model for

Diabetes Self-Management

The goal of this project is to develop a peer model of care to prevent and manage diabetes among Waukesha's Hispanic families.

Partners:

  • Center for Healthy Communities, Department of Family and Community Medicine, Medical College of Wisconsin
  • Hispanic Health and Human Services, Carroll College
  • Hispanic Community Health Resource Center, ProHealth Care

Goals of the Program:

  • Assess existing peer health promoter models to determine the most effective desin for a health promoter model for diabetes education, prevention, and self-management for local Hispanics
  • Design a culturally appropriate peer health promoter model of diabetes education, prevention, and self-management
  • Design a curriculum and training program for Hispanic diabetes peer health promoters
  • Pilot peer health promoter model
  • Prepare implementation plan

Project Period:

  • July 1, 2007 - June 30, 2008

Current Activities:

  • Research and review best practices in peer health promotion
  • Preliminary written report on best practices
  • Feedback from client stakeholders on report
  • Revise report of best practices including feedback from client stakeholders

Future Plans:

  • Research and review best practices in peer health promotion
  • Evaluate potential on-going stakeholder involvement
  • Design evaluation tool for model
  • Create a culturally-appropriate curriculum and training program
  • Test curriculum with Hispanic health promoters
  • Survey key client and professional stakeholders about the curriculum
