MyChart® is a free, easy and secure way to view your health information and communicate with your healthcare team at the primary care clinics of Froedtert & the Medical College of Wisconsin. MyChart is the shared patient electronic health record integrated with the Epic® electronic medical record used by the healthcare team at Froedtert & Medical College of Wisconsin clinics.
MyChart® is currently available to all patients of Froedtert & Medical College of Wisconsin ambulatory clinics that use the electronic medical record. With MyChart®, you can:
You enroll in MyChart® at your clinic. Simply ask for an enrollment form at the clinic reception desk. This form is also available online at http://www.mychartlink.com to print, complete and bring to the clinic. Once you enroll, you’ll receive a code to activate MyChart®. You then create your own username and password. Additional paperwork is required for access to family member health records and appointments.
MyChart® is not intended for sending messages requiring urgent attention. For urgent medical matters, contact your doctor’s office by phone.
