Institutional Biosafety Protocols Submission Forms

The following are links to forms included in the application process.  For research to be approved, you must provide responses for all applicable sections to the Institutional Biosafety Committee (IBC). 

IBC meetings are held on the second Tuesday of every month.  New applications need to be submitted at least two weeks prior to the meeting to ensure review.

Please note:

  • Individuals involved in studies utilizing human tissues, blood products, or other human materials must complete the blood-borne pathogen training and have a laboratory blood-borne exposure control plan in place. To find out more about the training, visit the Occupational Health Web site. 
     
  • Personnel requiring the use of N95 or N100 respirators must undergo fit testing and respiratory training prior to initiation of such studies.  Individuals should contact the MCW Occupational Health or Safety Departments to schedule an appointment. 
     
  • Studies requiring the shipment of biological materials must be done in accordance with current Federal regulations.  These regulations include mandatory training and certification for anyone shipping infectious agents, dry ice, or other hazardous material.  For more information about this training and certification, visit the Safety Department web site.

Please allow sufficient time for processing of your application.  It may take 30-60 days to obtain IBC approval.  Your protocol will be delayed if it is missing any required information.
