Patient Referral Instructions

For Medical Personnel:

Outpatient:
Please instruct the patient to call central scheduling (414-607-5280 or toll free 877-607-5280) for an appointment. Central scheduling will collect the demographic, history, and insurance information needed to set up the appointment. Central scheduling will contact our office when the information is ready, and we will call the patient back to make the appointment.

Inpatient:
The referring M.D. must enter a consult request through the Sunrise system.		   For Patients:
