1) Community Partners are broadly defined to be inclusive of statewide, regional and local partners. Multi-sector partnerships and multiple partners are welcomed.
Eligible community partners are non-profit, 501(c)(3) tax exempt organizations, and government organizations, including:
- health, social service, and other community-based organizations;
- faith-based organizations;
- state and local governments;
- scientific or professional associations, universities, schools;
- voluntary associations, foundations, civic and citizen groups; and,
- federally-recognized Indian tribal governments, tribes, or tribal organizations.
NOTE: For-profit companies and non-profit organizations that do not have 501(c) (3) tax exempt status may participate in the project, but funds from the Healthier Wisconsin Partnership Program will not be paid directly to these participants. A subcontractor or fiscal agent relationship with an eligible partner must be established so that funds are paid directly to the eligible partner.
2) Medical College of Wisconsin Partners are broadly defined to be inclusive of the full range of available staff and faculty expertise. Partnerships may also include faculty partners from multiple departments.
Each partnership must include at least one full-time or full professional effort faculty member at the Medical College of Wisconsin. This person will be considered the project's Principal Investigator.
NOTE: Staff, adjunct, or clinical faculty at the Medical College of Wisconsin may also participate as long as they serve in conjunction with a full-time or full professional effort faculty member. Medical College of Wisconsin medical student participation is welcomed. Student participation must be in conjunction with at least one full-time or full professional effort faculty at the Medical College of Wisconsin.
