The Medical College of Wisconsin Department of Physical Medicine and Rehabilitation clinics are located in Froedtert West, 2nd floor and Froedtert East, 5th floor.

How to get to the Medical College of Wisconsin campus:

From Sheboygan and Manitowoc
Follow I-43 south to Downtown Milwaukee and take exit 10 (I-94 west toward Madison). 
Take exit 305B, on US Hwy 45 toward Fond du Lac.  Exit Watertown Plank Road (exit 40).

From Racine and Kenosha
Follow I-94 north toward Milwaukee.  Veer left on exit 316 (I-894 bypass toward Madison and Fond du Lac).  Follow I-894 as it intersects with I-94.  Exit Watertown Plank Road (exit 40).

From Beloit and Northern Illinois
Follow I-43 to where it joins with I-894 and US Hwy 45 and go north towards Fond du Lac.  Exit Watertown Plank Road (exit 40).

From Fond du Lac
Follow US Hwy 45 south to Milwaukee.  Exit Watertown Plank Road (exit 40).

From Madison
Follow I-94 east toward Milwaukee.  Take exit 305B (US Hwy 45 toward Fond du Lac).  Exit Watertown Plank Road (exit 40).
