The use of radiation and radioactive material at MCW and FMLH must be done with the prior approval of the Radiation Safety Committee.  Investigators wishing to apply for authorization to use radiation or radioactive material should read the Standard Operating Policies, complete the appropriate form from the list below and submit the application to the Office of Radiation Safety.

Application Forms - Research (non human)
Authorized User Application.doc	Application for Authorization for the Use of Radioactive Materials - Use this form if you are a new user, or a previously aproved user starting a new project where RAM use is substantially different from previous use.  Contains the Application, AU Training Form and Instructions.

Amendment.doc	Existing Authorized Users may use this form for minor changes to authorization - new locations of use, increase possession limit, etc.
RAD Worker Training.pdf	All staff and students working directly with radioactive materials must have a completed copy of this training form on file with ORS.
Irradiator Access Form	Must be completed by any individual who needs access to MCW Irradiators, Radiobiology or Gamma Knife
