The purpose of this survey is to examine the organizational structure of the public health workforce by first characterizing the primary leadership styles of Local Health Department Directors as defined by Bolman and Deal's Four Frames: Structural, Human Resource, Political, and Symbolic.

The Bolman and Deal Leadership Orientation Instrument (LOI) is a 32 item survey and can be used to assess one’s own skills or by colleagues to assess their leaders skills. The tool has been used extensively in higher education institutions and has more recently been implemented in other areas including healthcare and health information management.(Hughes, 1990; Hughes, 1990; B. Sasnett & Clay, 2008b; Schneiderman, 2004; Schneiderman, 2005) To date, this multi-frame model has not been used to examine the organizational leadership in Local Health Departments.
