Forms To Be Completed By New Faculty
As of April 1, 2013 all credentialing applications are accessed, completed, signed and submitted electronically.
If credentialing is required for your appointment at MCW you will be receiving 2 emails from MCW Managed Care. The first email will include the link to your application and the second email will include your password. If you have not received an email from MCW Managed Care and you need to complete credentialing, please contact the department via credentialing@mcw.edu or via phone at (414) 955-7101. Froedtert and Children's Hospital both have separate credentialing forms.
Do you need to apply for privileges at Children’s Hospital of Wisconsin? If so, please go to chw.org/staff, Milwaukee, user code: medstaff and password: mss2010 to fill out the request for an application.
Do you need to apply for privileges at Froedtert Hospital? If so, please contact the credentialing office at (414) 805-3040 or cvs@froedterthealth.org to request an application.
