Referring a Patient

The Division of Gastroenterology & Hepatology is staffed by a nationally recognized group of physicians and scientists representing multiple specialties. Their unique collaboration provides a multi-faceted approach to patient care, research and education in the prevention, diagnosis and treatment of all forms of digestive disease.

If you are an EPIC user, please send a referral through the EPIC system.

If you are not an EPIC user, you may:

1. Download the appropriate referral form (see below)
2. Complete the form making sure to include all patient demographics
