DISCLAIMER: You are encouraged to discuss your interest in conducting a global health elective with your faculty mentor and MCWAH prior to pursuing the below steps.

MCWAH Administration Requirements:
Please review the MCWAH Institutional Policy for Offsite Rotations and submit the following MCWAH forms to MCWAH at least 60 days prior to the rotation:

• Letter of support to to Dr. Simons (stating why the rotation is necessary and why the experience cannot be obtained at MCW)
• Letter of support to Dr. Simons from the on-site preceptor (citing his/her support of your rotation as well as his/her qualifications)
• Request for Elective Rotation
• Elective Rotation Program Letter of Agreement (PDF)
• Release and Waiver of Liability Form (PDF)
