Health Insurance Information/Forms

HEALTH INSURANCE

Health insurance coverage is a requirement for all full-time graduate students.  If you have been admitted as a full-time PhD degree seeker you are offered enrollment in the Humana Health Insurance, Anthem Dental Insurance and Professional Vision Services Plans.  You may exercise the option to add a spouse and or children with the additional cost of coverage deducted from your monthly pay. 

The annual premium for single coverage is paid by the college for all full-time students in the PhD programs as part of their fellowship program.  Some students are covered under health insurance policies of their parents, spouses or full-time employer and are eligible to be waived from enrollment.  Students have also elected to require single coverage for dental and vision for all full-time students enrolled in the health insurance plan.  Full time degree-seeking MS, MA or MPH graduate students may purchase the student health insurance plan.  Each student is responsible for payment of this coverage at registration or through stipend deduction

STUDENT HEALTH SERVICES

The Medical College of Wisconsin wishes to insure that all students have access to excellent high-quality health care. If your Primary Care Physician (PCP)is located at Student Health Service, you may utilize its services.  The Student Health Service is located at Froedtert Hospital's Department of Internal Medicine.  Guidelines of the graduate student insurance plan must be followed to ensure the highest level of payment coverage for medical services. A clinic card is required for students and dependents which may be applied for through the Student Health Service.  The Clinic card is absolutely necessary in order to obtain services.  To make an appointment, call internal medicine 805-6644.

If you need a doctor outside the normal hours please call (414) 507-9589 which is available 24 hours a day/7 days a week.  The location is on the corner of Watertown Plank Road & Highway 100 (Tosa Center, 1155 N. Mayfair Rd., Milwaukee, WI  53226).

Immunizations and necessary testing is provided and immunization records are maintained in the Occupational Health Administrative Area at the Curative Building. These services are available only to full-time degree-seeking students.

STUDENT MENTAL HEALTH SERVICES

The College provides access to confidential mental health services (24) twenty-four hours a day, (7) seven days a week.  Under the supervision of a medical director, students will be evaluated by faculty psychiatrists, senior residents in psychiatry, or senior psychologist.  Students receive (8) eight free sessions of Medical College mental health services, per academic year, for free.  Most major medical insurance policies should cover some of the cost after these initial eight sessions. 

The student Mental Health Services office is located off site for confidentiality purposes.  These services are available only to full-time degree-seeking students.  Students are welcome to call (414) 955-8933.

 COBRA INSURANCE

(Consolidated Omnibus Budget Reconciliation Act)

When a graduate student completes his/her studies in the Graduate School of Biomedical Sciences, student has the opportunity to continue his/her current insurance coverage for a period up to eighteen (18) months the length of COBRA Insurance.  The individual selecting to continue his/her insurance coverage in this manner is responsible for all of the payments related to the COBRA insurance.  (Current monthly premium costs is given to the student at that time). Student must complete the form, sign, date and return the form to the Grad School.

If student continues his/her insurance coverage via the COBRA insurance, he/she will need to submit a check by the 1st of the month, for the premiums covering that month.  The check should be made payable to the Medical College of Wisconsin and mailed to or bring to grad school - Third floor - H3235.

Student will have a thirty (30) day grace period in which to make the payment, if payment has not been received at the Graduate School of Biomedical Sciences the insurance coverage will be terminated for non-payment.
