    Project Goal Statement: This project aims to increase the accessibility of primary and preventive health services to children having developmental delays, disabilities, or complex medical and social issues, from culturally diverse low-income families.  The project will also create a continuity of care between the primary physician and community service providers.

    Award Amount: $25,000
