Project Goal Statement: The GAMP Chronic Disease Management Initiative will utilize case management to improve patient self-management skills, improve health outcomes, and increase the effective use of health resources for 625 indigent, uninsured persons suffering from chronic disease and establish a model for replication throughout the GAMP system.
