    Project Goal Statement: CCDM will effect prevention of complications by screening for/managing hypertension (HTN), diabetes (DM) and obesity (O) at churches and food pantries that serve Milwaukee's poor African Americans and by promoting low-cost medicine, nutrition, education and exercise in a tiered community model.
