     Project End Date: 12/31/2013 - No Cost Extension Through: 4/30/2014

Project Goal Statement: To train a cadre of Lay Trainers who are themselves parents of children and youth with special health care needs (CYSHCN) to teach care coordination skills and behaviors to other families of CYSHCN using a recently developed care coordination curriculum.

Award Amount: $200,000
