Health Improvement Priorities

HWPP provides funding for health improvement through three interrelated health improvement priorities. By focusing on these, projects will build capacity that will endure well beyond HWPP funding. The priority areas identified by HWPP are intentionally broad and rely on the creativity of partners to propose innovative programs to improve the health of the public.

HWPP projects must address health improvement by first selecting one primary priority area from each of the three sections below on which to focus. Projects may further refine that focus by identifying one or more additional categories from each area that may also be affected by the project activities.
 

1. INFRASTRUCTURE FOCUS AREAS TO TRANSFORM HEALTH IMPROVEMENTS
Projects should address one or more of the the Infrastructure Focus Areas to lead to transformative health improvement:

• access to high-quality health services
• collaborative partnerships for community health improvement
• diverse, sufficient and competent workforce that promotes and protects health
• emergency preparedness, response and recovery
• health literacy
• public health capacity and quality
• public health leadership and policy
• systems to manage and share health information and knowledge

 2. POPULATIONS AFFECTED
Projects should address one or more of the following specific populations:

• children and adolescents
• disabled
• men
• racial and ethnic populations
• rural
• seniors
• uninsured
• urban
• women

3. HEALTH FOCUS AREAS
Projects should address one or more of the following:

• adequate, appropriate and safe food and nutrition
• alcohol and other drug use
• cancer
• chronic disease prevention and management - note specialization in one of the following areas:

arthritis     asthma     diabetes     heart disease     hypertension     stroke     other __________

• communicable disease prevention and control
• environmental and occupational health
• healthy growth and development
• injury and violence
• mental health
• oral health
• physical activity
• reproductive and sexual health
• tobacco use and exposure
• other (with specification)
