This summary list includes faculty and staff from the Medical College of Wisconsin who have indicated interest in partnering with community organizations to submit proposals to the Healthier Wisconsin Partnership Program.  Each have indicated their fields of expertise and interest that are linked to the Health Improvement Model.  Areas that are not directly defined in the Health Improvement Model but are viable for project development have also been listed as "Other Expertise or Interest." 

Please note that this list is not an exclusive representation of potential academic partners.  Any Medical College of Wisconsin faculty member OR staff member serving in conjunction with faculty is eligible to serve as a partner.

Interested in partnering with  a community based home care agency in the greater Milwaukee area who would partner with us to increase education of both physicians and home care providers. The partner would bring a knowledge of home care processes, patients, and its clinical providers and patient base. We would bring knowledge of inpatient/outpatient clinic and physician roles. The community partner would also provide a location for expanded involvement in student education, with opportunities for student physicians, nurses, and social workers to participate in the care of frail elderly patients in the home setting. The end goal of the project would be a community of home-bound elders who would receive excellent, coordinated care from physicians and home care agency providers in our area.
