Requirements

Standards for the use of Healthier Wisconsin Partnership Program funds disseminated through the Request for Proposals (RFP) are based on the following two required components:

1. Health Improvement Model: Proposals must interrelate health promotion, disease prevention, specific populations, and the transformation of health improvement efforts.
   
2. Community – Academic Partnership Model: Proposals must be submitted by partnerships comprised of at least one MCW partner and one community organization partner.
