COMMUNITY-BASED CHRONIC DISEASE MANAGEMENT (CCDM)
This project was featured in the May 2, 2008 edition of The Business Journal in the article, “Improving Milwaukee’s Health Block by Block,” as well as in Advance For Nurses, “Milwaukee nurses screen food pantry visitors for chronic diseases.” Both articles outlined the efforts of this HWPP project to utilize nurses to screen people for tobacco use, obesity, high cholesterol, diabetes and hypertension at the New Life Food Pantry in Milwaukee.
MILWAUKEE ALLIANCE FOR SEXUAL HEALTH (MASH)
This project was featured in the May 21, 2008 issue of the Shepherd Express in the article, “The Silent Epidemic.” The article discusses the alarming high rates of teens with STDs in Milwaukee compared to all other cities in Wisconsin. It addresses why the epidemic is so prevalent and what this project is doing to improve the situation.
STRENGTHENING PUBLIC HEALTH POLICY MAKING FOR A HEALTHIER MILWAUKEE
The partnership produced a report outlining the process used to assess City of Milwaukee Health Department’s policymaking capacity and the development of a framework for action. The hope of the partnership is that this report will inspire other Wisconsin health departments to embrace the tool of evidence-based policymaking and engage in community-academic partnerships to achieve healthier communities.
RIVERWEST HEALTH INITIATIVE
Jim Sanders, MD, MPH (Medical College) and Mary Jo Baisch, PhD, RN (UW-Milwaukee) had an article published in the spring 2008 edition Progress in Community Health Partnerships: Research, Education, and Action. This is a publication of The Johns Hopkins University Press. The title of their article is, "Community-Based Participatory Action: Impact on a Neighborhood Level Community Health Improvement Process."
