Ask-A-Librarian

MCW Librarians are available for assistance via chat, phone, email, text, or in person. The Todd Wehr Library Reference Desk is staffed Monday - Friday, 9 a.m.-4 p.m., but skilled staff are available between 8 a.m. and 5 p.m. Monday - Friday (excepting holidays) for in-person and phone help.

Form for submitting purchase suggestions to the MCW Libraries. MCW/CHW/FH users only.

Photo Use Application Form

MCW Libraries Use of Photos Application. Use this form to request access to archival photos maintained by the MCW Libraries.

Poster Printing

Poster Printing is available from the Library for students and departments. Fees and timetables do apply.

Proxy Access Request

Form for volunteer clinical faculty and adjunct faculty to request access via the proxy server.

VCCR Module Request Form

The Society of Critical Care Medicine has two online courses call Virtual Critical Care Rounds. Virtual Critical Care Rounds (VCCR) are designed to better prepare residents and students to contribute to the diagnosis and management of the critically ill patient during their intensive care unit rounds. MCW, Froedtert and Children's Hospital of Wisconsin personnel may request access to the course modules by completing this form.
