This Web-based tool from the U.S. Surgeon General helps you and your family track your health history. Step-by-step directions lead you through the process. You can save the history and add or change information as you learn more.

 American Medical Association Health History Information

Use this section of the American Medical Association Web site to fill in health history information. After filling out an online form, the health history information can easily be printed and brought to a doctor's appointment. Forms include a prenatal screening questionnaire, a pediatric clinical genetics questionnaire, and an adult family history form.

 National Society of Genetic Counselors Family History Information

This useful Web page includes detailed directions about creating a health history family tree. There are tips on what to include and how to gather information. Also included is a link to a diagram showing how to record the information in a concise and useful way.
