• Wisconsin Residency Form  If you are a Wisconsin resident or are going to claim residency this form is to be completed and forwarded to the Higher Educational Aids Board in Madison, WI.
• Dean's Certification If you indicated on the AMCAS or our Secondary Application that an institutional action had been taken against you this form is to be forwarded to the appropriate office at the institution with an explanation for verification.  It will then be forwarded to us.  Please know that if an action is indicated this form is required to complete the application.
• Technical Standards  Technical Standards consist of the minimum physical, cognitive and emotional requirements to provide reasonable assurance that candidates or students can complete the entire course of study and participate fully in all aspects of medical training.
