Graduate Student Forms
Every student who wishes to receive financial aid at the Medical College of Wisconsin must complete and submit the following forms. Unless otherwise instructed, return all application materials to the Financial Aid Office at the Medical College of Wisconsin. The notable exception is the Free Application for Federal Student Aid. You will file this form on line using your Pin # as your signature. Paper copies of all forms listed can be obtained in the Financial Aid Office.
Free Application for Federal Student Aid (FAFSA), electronic or Paper
This form is used to calculate your expected contribution and provide demographic and financial information. You can use estimated information when filling out this form. Financial aid applications are evaluated on the basis of income, assets, family size and other relevant information for the student/spouse. This form is available at www.fafsa.ed.gov. Our school code is 015611. We recommend that your FAFSA be submitted to the Central Processor by June 15, 2013 for fall term and November 1, 2013 for spring term. Electronic transmission will be accepted beginning January 1, 2013.
Financial Aid Application (FAA)
The FAA is an internal form developed by the Financial Aid Office at the Medical College of Wisconsin. This form provides additional information that is not on the FAFSA. Print, complete and mail this form to the Financial Aid Office.

Students wanting to apply for aid for the 2012-2013 school year or summer 2013 should contact the Financial Aid Office for the appropriate forms.
SIGNED PHOTOCOPY OF 2012 FEDERAL INCOME TAX RETURN
Please submit a signed copy of your 2012 Federal tax return, including all schedules. If you or your spouse did not and will not file a 2012 Federal Income Tax Return, a non-tax filer statement must be completed and submitted to our office.

A copy of your 1040, signed and dated, must be attached to the printout of your electronic filing.

SPOUSE'S NON-TAX FILER STATEMENT
If you are married, your spouse is required to submit a non-tax filer statement or a signed copy of the 2012 Federal Income Tax Return your spouse filed.
DOCUMENTATION OF ANY ADDITIONAL AND UNUSUAL EXPENSES
This is defined as expenses, other than allotted school costs, you may incur during the school year. Acceptable expenses include costs such as daycare, car repairs or medical expenses that go beyond the budget amount (moving expenses and security deposits are not allowed).
THIS ADDITIONAL FORM IS REQUIRED IF YOU ARE A NEW GRADUATE STUDENT
COPY OF RESIDENT ALIEN VERIFICATION CARD (GREEN CARD)- Please bring your green card to the Financial Aid Office and we will photocopy the card for you. If you become a naturalized citizen while in school, please provide our office with a copy of your papers.
