C/D: OD 0.35; OS 0.40 Neovascularization of the optic disc(NVD) OD, No NVD OS Vitreous: No Vitreous Hemorrhage OU Optic Nerve: C/D 0.35 OD and 0.4 OS, NVD involving ~ 50% of disc OD, No NVD OS Macula: Flat with multiple microaneurysms and hard exudates > 500 microns away from the fovea OU. No Clinically Significant Macular Edema (CSME) OU Peripheral Retina: Attached 360 degrees in both eyes. Multiple dot-blot hemorrhages in the periphery OU. Cotton Wool spots OS Focal areas of NVE OS.
