Columbia-St. Mary’s is partnering with the Christian Faith Fellowship Church to create a community chronic disease management clinic on Milwaukee’s northwest side. The clinic is modeled on similar clinic partnership between MCW and Columbia-St. Mary’s. The first community chronic disease management clinic was developed by the New Life Food Pantry through funding from MCW’s Healthier Wisconsin Partnership Program.
