TEAM (Together Everyone Achieves More) Medical Home Training

The American Academy of Pediatrics defines Medical Home as accessible, family-centered, continuous, comprehensive, coordinated, compassionate and culturally competent. A core outcome of Healthy People 2010 and many national organizations is that all children have a Medical Home. Multiple surveys indicate that physicians do not feel residency prepared them to translate the philosophy of Medical Home into an effective systems based practice for Children and Youth with Special Health Care Needs (CYSHCN).

To address this, the Medical College of Wisconsin, Children’s Hospital of Wisconsin and the Southeast Regional Center for CYSHCN developed TEAM Medical Home Training, a required curriculum spanning all three years of pediatric residency. TEAM improves health and quality of life by preparing future physicians to effectively provide Medical Homes to CYSHCN. The emphasis is on systems based care and the coordination of community based, tertiary and primary services focused on meeting not only the child’s medical but also their psychosocial, educational, recreational and emotional needs. Year 1 and 2 residents are introduced to Medical Home, CYSHCN, and family impact though: home visits, discussion groups, noon conferences, and workshops. Year 3 focuses on translating this into practical application during a block rotation.
