Special Needs Program

The Special Needs Program provides care coordination and co-management of children with multiple chronic complex medical conditions. Our goal is to ensure seamless inpatient and outpatient care for this population of medically complex and fragile children in partnership with their families, primary care physicians, specialists, community providers and others involved in their care.
