The Geriatrics Ambulatory Team works closely with Geriatrics Memory Care and Primary Care Patients and their families regarding symptom management and future planning especially for concerns such as memory enhancement, behavior management, living arrangements, and community services.

Geriatrics Memory Disorder Patients

Geriatrics Memory Disorder Patients will receive comprehensive care through the stages of their memory disorder, beginning with an initial cognitive evaluation to determine or confirm a diagnosis, which includes:

  • Comprehensive medical history
  • Physical exam
  • Labs and imaging
  • Memory testing

Geriatrics Primary Care Patients

Geriatrics Primary Care Patients will receive comprehensive care in a unique primary care setting in order to optimize overall health and enhance quality of life in patients with symptoms such as:

  • Chronic illnesses
  • Cognitive decline
  • Frailty
