Employer Information

Your employer is the Medical College of Wisconsin Affiliated Hospitals (MCWAH), not the Medical College of Wisconsin (MCW).

This distinction is important because MCWAH employees have different benefits, conditions, and terms of employment. When you complete your training, all GME verifications should be directed to the Medical College of Wisconsin Affiliated Hospitals (MCWAH).

Please monitor your personal email for important messages that contain links and time sensitive requests. The information provided on this page will summarize your responsibilities and when you can expect messages and deadlines throughout the spring and early summer. The twelve (12) onboarding steps described on this page are required to become a MCWAH employee and must be completed before you can start your MCWAH training program.

Missed deadlines may result in a delayed start which may impact your overall completion date.

Onboarding Overview

  • All onboarding materials will be emailed to the personal email address provided by your MCWAH program coordinator.
  • You will receive additional emails for licensing, ePDMP enrollment, required course work, and individual hospital onboarding processes that will require your attention/completion by the due dates listed within those messages.
  • MCWAH is part of the MCW Information Technology Solutions (ITS) network; therefore, you will receive an MCW username/password and @mcw.edu email address. After May 15, all emails will be sent to your @mcw.edu email address, your official address while you are a MCWAH trainee.
  • A half-day MCWAH New Housestaff Orientation will be held online at the end of June. An Outlook invitation will be sent to your MCW email address with the virtual meeting link and supporting documentation/resources. Please note, the MCWAH offices will be closed to in-person visitors during the virtual orientation session. All in-person business, such as I-9 verification, must be completed either before or after the orientation session.

Important Onboarding Communication Dates

April 1

1) Email from gme_forms@mcw.edu via DocuSign to your personal email address:

  • Contains full list of of steps for onboarding process
  • Details and DocuSign link to MCWAH Acknowledgement Form packet, which includes:
    • Disclosure and authorization regarding procedurement of consumer reports and investigative consumer reports
    • Acknowledgement and request to release criminal background information
    • Patient Information privacy and security information and Outlook Application Memo
  • Internal Occupational Health: record sharing agreement

2) Email from Qualtrics to your personal email address:

  • Complete MCWAH Employment Application
    • The form will take longer than 60 minutes to complete.
    • Required materials are listed in Step 2 below as well as in the April 1 email attachment.

3) Email from Qualtrics to your personal email address:

  • Internal Occupational Health (IOH) Packet (2-step process)
    • Complete IOH immunization registry and Fit Testing questionnaire
    • Download, print and complete the Provider Verification form that was attached to the April 1 email from gme_forms@mcw.edu (see above).

4) Email from DocuSign to your personal email address:

  • Two-step Medical School Graduation Verification process
  • Must be forwarded to your medical school of graduation by May 15
  • Medical College of Wisconsin or UW Madison School of Medicine and Public Health and International Graduates are exempt from this step.

  • Sign and submit MCWAH Academic Year Contract

  • Log in to Workday to verify required employee data (Step 9 below)
  • Visit the MCWAH offices to complete your in-person I9 verification. This must be completed before the end of your third day of training.

Post Onboarding (variable due dates)

  • For those with previous GME training and/or clinical practice, a Docusign request will be sent to you for previous GME verification and PLI claims history confirmation. Please reach out to kreinke@mcw.edu with questions.
  • Create your online Transamerica account and verify your deferrals and any additional changes you would like to make to your 403(b) account. Please reach out to gme_retirement@mcw.edu with questions.

Step 1: Complete MCWAH Release of Information & Patient Privacy Forms

Follow the instructions provided in the April 1 email from gme_forms@mcw.edu to complete your release of information and patient privacy forms.

Step 2: Complete MCWAH Employment & Background Check Form

A personalized link will be provided in the April 1 email from gme_forms@mcw.edu to complete your employment application and background check forms.

The application form is long and will take more than 60 minutes to complete. If you cannot finish the application in one session, your personalized link will be active for seven (7) days after you access it the first time.

You will need the following information available to complete the form:

  • Full Social Security Number (if not available, which is unusual, the form will direct you accordingly)
  • Emergency contact name and address/phone information
  • Education history
    • Medical School
    • Advanced Degrees (PhD, MPH, etc)
    • Graduate Medical Education (all residency or fellowship training) with accreditation confirmation
  • Employment history
    • Research experience after medical school and/or during GME training (to account for gaps in training)
    • Employment/clinical work information after medical school and/or during GME training (to account for gaps in training)
  • Citizenship information
    • International medical school graduates will need to submit their ECFMG number and issue date
  • Wisconsin Caregiver Background Check History
    • Disclosures for criminal and licensing charges, convictions, revocations, restrictions, etc.
    • Non-Wisconsin locations you have lived in the past seven (7) years
  • Licensing and NPI
    • Step/COMLEX Exam completion
    • Medical License numbers (if applicable) for both Wisconsin and out-of-state licenses
    • NPI number registration
  • Current headshot (either JPG or PDF format) uploaded directly to the form.
  • Driver's license, state issued ID or passport (scanned and saved as a JPG or PDF) uploaded directly to the form.

Step 3: Complete Internal Occupational Health (IOH) Packet

Follow the instructions provided in the April 1 email from gme_forms@mcw.edu to complete your IOH packet.

You must be fully cleared by IOH before you can start in any clinical setting. A physical and physician sign off is a required part of the medical clearance process.

It is highly recommended that you keep copies of your immunizations. MCWAH does not maintain copies of immunization records; you must work directly with the Internal Occupational Health Team.

Step 4: Complete & Forward Registrar Confirmation Form

Follow the instructions provided in the April 1 email from Kelly Reinke (kreinke@mcw.edu) via DocuSign to complete your Registrar Confirmation Form.

Please complete the two-step Medical School Graduation Verification process and forward to the Registrar’s Office of the medical school you graduated from.

Step 5: Login to MCW Email & Verify VPN Access

Follow the instructions provided in the May 15 email from gme@mcw.edu on how to login to the MCW email system and access VPN when you are off campus.

Information will be emailed to both your personal and MCW email addresses. Please verify you can access both systems.

After May 15, all correspondence from MCWAH and affiliate hospitals where you will train will be emailed only to your MCW email address. This will be your official address while you are a MCWAH trainee. MCWAH maintains personal email addresses for emergency use only.

Step 6: Complete Required Online Courses

Follow the instructions provided in the May 15 email from gme@mcw.edu (subject: Required Courses for Incoming Housestaff) to access your required online courses.

Each year MCWAH requires incoming housestaff to complete designated mandatory courses. The courses are found on two separate platforms: Desire2Learn (D2L) and Institute for Healthcare Improvement (IHI).

All courses must be completed prior to starting in your GME training program.

Step 7: Complete Benefits Enrollment via online Employee Navigator

Follow the instructions provided in the May 15 email from gme_benefits@mcw.edu to access the online Employee Navigator to enroll in Health, Dental, and Vision insurance benefits. Completion of the Beneficiary Designation Form will also be required.

Note: If you choose not to participate in MCWAH benefits, you must WAIVE coverage in Employee Navigator.

You will be automatically enrolled in the following group plans:

  • Life Insurance
  • Long Term Disability Insurance
  • Short Term Disability Insurance
  • AD&D
  • MCWAH 403(b) Plan via Transamerica

Step 8: Sign MCWAH Academic Year Contract

Contracts will be emailed from DocuSign to those who have completed Steps 1-4 (above).

Missed deadlines and/or delays in submitting required onboarding materials may result in delays of contract generation, which could impact your start date and potentially your overall completion date.

Step 9: Verify Required Employee Data

Beginning June 15, log in to Workday to ensure the following data is correct/entered:

  • Current (Wisconsin) Mailing Address
  • Contact information
  • Emergency Contact
  • Personal email address (not university email address)
  • Direct deposit (bank) information
  • Federal and State Tax Withholdings (W4)

All tax and direct deposit information must be entered by July 1.

Step 10: Attend June 30 Virtual MCWAH Orientation

The virtual MCWAH Orientation is scheduled for June 30, 8 a.m. to 12 p.m. The MCWAH office will add the orientation to your Outlook calendar.

During orientation, the MCWAH office will be closed to in-person needs to allow all incoming trainees the opportunity to attend.

Reminder emails and a detailed agenda will be emailed to your MCW email address.

Step 12: Post-Onboarding Activities

Create Online Transamerica Account (must be done AFTER July 1)
A welcome email with newsletter and instructions on how to set up an online profile for your MCWAH 403(b) Retirement Plan will be emailed to your MCW email address. You will be automatically enrolled at 3% of your bi-weekly salary; this can be changed after you create your online profile. All MCWAH housestaff will receive a total non-elective contribution (paid directly by MCWAH) of $1200 for the academic year. Please reach out to gme_retirement@mcw.edu with questions.

Housestaff with Previous GME Training and Clinical Work
On August 1, a request will be emailed to your MCW email address via DocuSign to confirm previous GME training and PLI insurance coverage specifically for physician practice of medicine coverage. Please complete the request in a timely fashion; your program director and coordinator will be made aware if GME training and PLI verifications are not received.
