Please complete the online application and upload your CV/resume via the link below. In order to be considered for a MCW Affiliate Clinical Instructor appointment, you must submit both a preceptor application and an up to date CV or resume. Preceptors who submit both documents will also be given access to CORE ELMS, the MCW Pharmacy School's electronic learning management system. This system houses our rotation registration system as well as preceptor orientation materials. Preceptor applicants who do not submit both documents will be contacted by a member of the Office of Experiential Education.
